Request for Assistance online form
This form is for children and young people aged 5 years and above only.
The RfA form can be saved mid-way if required.
We encourage schools to lead on Requests for Assistance, but other professionals (e.g. GPs, Health Visitors) are also welcome to submit requests, provided all required information is included.
Consent
All referrers must complete the mandatory consent statements on the online Request for Assistance form. These statements confirm that consent has been discussed and obtained before the request is submitted.
- Young people aged 16 years and above are required to provide their own consent.
- Children and young people under the age of 16 require consent from a parent or carer.
- Young people aged 12 years and above are generally expected to provide their own consent, in addition to parental/carer consent where required, to ensure their views are included.
Information for Schools
When a Request for Assistance (RfA) is submitted by a school, a signed consent form is required in additional to the consent statements on the RfA.
Schools must:
- Obtain a signed consent form, and
- upload the signed consent form to SEEMiS Wellbeing Application.
Please note, schools no longer require to upload the signed consent form to the Single Point of Access (SPA).

Single Point of Access Consent Form for schools
Child Plan
If a comprehensive Child’s Plan is already in place, completing a RfA should take no more than 5 minutes. The Child’s Plan can be uploaded during the process.
Mental Health and Wellbeing Locality Forums
The locality forums aim to provide an additional level of support for children and young people who are experiencing difficulties with their mental health and wellbeing, where their needs cannot be met through universal supports, or through a single agency intervention (e.g. School Counselling Service, The Exchange).
Forums meet monthly and include professionals from:
- Education Inclusion and Equalities Team (Principal Teachers)
- Educational Psychology
- Social Work
- Allied Health Professionals
- Representation from a local third sector organisation
Requests for mental health and wellbeing supports should come through the SPA when:
- Multiple or complex concerns are present that require the support of more than one agency or service
- School counselling has been offered but concerns remain
What happens once a RfA is submitted?
- RfA submitted and an acknowledgement email is sent with a reference number. All RfAs are screened weekly. Referrers should use this reference number to support any correspondence.
- If all information is present, RfAs will progress to either the NDD triage panel or the appropriate locality forum. If not accepted – referrer will be informed, with explanation. In some cases, the RfA will go to both NDD triage panel and the Locality Forum.
- Referrers, and where appropriate parents/carers, will be updated on outcomes and recommendations from the Locality Forums, and updated throughout the assessment process for NDD.
