The group gave consideration to a report by Craig McNally, Senior Health Promotion Specialist (Alcohol & Drugs).  The report detailed information and a number of inter-related strands to ensure that individuals and communities have access to appropriate information, knowledge and skills which enable them to make positive, well informed, choices regarding their alcohol use.

 

The Chair thanked Craig McNally for his informative update.

 

Christina West had also submitted a MAKI Locality update

Christina West, Locality Manager MAKI  had intimated her apologies and had been unable to field an NHS representative in her place.  However, she had forwarded an update which was tabled at the meeting and to which, Charles Reppke Head of Governance and Law spoke.

 

1.      Background and Summary

 

This paper updates on issues previously reported at MAKI LACPiG meetings and highlights recent locality developments/issues.

 

2.      Report

Reshaping Care for Older People

Reshaping Care for Older people is the national policy launched in 2011 by the Scottish Government to address the demographic changes that health and social care services need to respond to in the coming years. Below is an update on locality specific areas of work which are currently in progress. These have been identified by local groups as priority areas for development.

Islay and Jura

 

  • Looking at ways to permit social work and health staff to drive vans to transport necessary equipment such as hoists out of hours when alternative would be admission to hospital
  • Physiotherapy has been meeting with the social work team weekly and has now been joined by Community Nurse Team Lead to discuss cases.   As well as being positive for the patient, it allows everyone to get to know each other and improves team relationships
  • Although Carefirst has not been installed in the hospital, a social work presence in the hospital will be implemented half day per week from w/c 28.10.13
  • Potential for Personal Outcome Plan assessment tool to be used as a referral tool by nursing staff being explored
  • A leaflet was produced detailing summer activities for children. The idea could be extended for older people’s activities. A regular interest column in the Ileach for older people has been proposed.
  • The structure of the group is changing.   Plan to rotate chair in Islay & Jura.
  • The sub group drafted a bid for increasing access to activities for older people.  The role of the coordinator employed by Dochas will be extended to co-ordinate transport, events, socialising etc. for older people on Islay

 

 

Kintyre

 

  • Personal Outcome Plans are being undertaken by both health & social work staff
  • Regular Emergency Admission meetings taking place to review whether or not an admission to hospital was necessary or could have been avoided if specific services were available
  • Anticipatory Care Plans are being generated often informed by emergency admission meeting
  • Falls & reablement training underway and open to all partners. Post holder in post for 12 months to progress across MAKI locality
  • Community team has worked a pilot of extended hours. Plans underway to make this more robust and permanent.
  • A partnership bid has been submitted for grant funding of £75k to create a dementia friendly community ( health, Social Work & 3rd Sector)
  • Tele care alarms now established as part of care packages
  • Co-location of adult care and community nursing is advancing. Council at present evaluating accommodation available.
  • Work planned to look at day care in a community setting, improved transport and one stop carers support .

 

Mid Argyll

  • Winter is Here Event - information, advice, goodie bags, cafe, partnership across agencies to take place Friday 6th December in Lochgilphead community centre.
  • Great outputs from RCOP Locality Day, fantastic attendance and engagement- being pulled into a new action plan with identification of key priorities for the group being agreed
  • Attendance at RCOP Leadership events by members of group.
  • Rolling our reablement and fall prevention training across the area.
  • Extended working hours from 08.30-20.30 hrs by the extended community care team to commence 2nd December.

Additional Information

Kintyre

·         Mr Duncan has moved into the Campbeltown dental access centre and is now fully operational in these new facilities.

·         A new GP has been recruited for Kintyre Medical Group (Muasdale, Southend and Gigha) to commence Monday 6th January 2014.

 

 

Islay & Jura

 

·         The work of the Islay Review Steering Group concluded at the end of November following receipt and endorsement of Professor Peterkin’s final report and recommendations. Establishment of an Islay Implementation Group and a draft action was agreed at Steering Group meeting on 29th November. The recommendations of the Steering Group will now be tabled at CHP Committee in December 2013, in order for implementation to commence January 2014.

 

 

Mid Argyll

 

·         The refurbishment programme to provide a day treatment unit and relatives room within MACHICC have been completed and an official opening is due to take place on 9th December attended by Michael Matheson, Cabinet Secretary for Public Health and Mike Russell, local MSP. Working in partnership with MacMillan Cancer this refurbishment will enhance the environment for patients by providing a dedicated treatment and relaxation space for three patients. A relative’s area has also been developed within the hospital, providing an area of privacy and relaxation for families to stay when relatives are receiving end of life care in the hospital.        

·         The Inveraray/Furnace GP vacancy will be advertised in the next few weeks.

Christina West detailed ongoing recruitment problems in the Radiotherapy Department.  She informed the group of a Clinical Service Review on Islay and an update on the work being undertaken by the Chemotherapy Service. 

 

She updated the group on the creative marketing being progressed by Furnace CC in the recruitment of a GP. 

 

Christina advised of a recent meeting held in Campbeltown to address the bed closures and proposal for a Methadone clinic.

Update from Christina West, Locality Manager

 

Kintyre

 

·         A further bed modelling meeting was undertaken on 10th January and agreement reached that Campbeltown Community Hospital will operate with a total of 21 beds from 1st April, this comprises 19 acute GP beds and 2 continuing care beds for existing patients. In addition it was agreed that there should be a contingency plan to make a further three beds available at short notice should the need arise. A redesign group, led by Donnie Cameron, Clinical Services Manager, will now identify and progress the required actions to implement the changes.

·         To address concerns raised by the Community Council regarding the decisions made at the bed modelling meeting in January, a drop in event has been organised to take place on Thursday 14th March from 2-8pm in the Town Hall Campbeltown. The information available at the bed modelling will be available for wider community review. Members of the public are invited to drop in at any time and meet with local NHS Highland staff to discuss the recent changes to care provision within the hospital and the community. In addition to staff from the NHS and Social Work there will also be representatives from a number of other organisations who provide services in the community such as Carr-Gorm, the Red Cross and Tele Care. The event will be advertised through the local paper, radio and the distribution of posters

 

Islay & Jura

 

·         At a meeting on 16th January, members of the CHP management team and the Islay GPs agreed a time limited (12 months) package of additional support to ensure the continued safe delivery of health services on Islay.

·         Dr Gordon Peterkin has been appointed to the role of independent clinical expert to support the Review of Health Services on Islay.

·         The first meeting of the Islay Review Steering Group took place on Friday 1st March, chaired by Robin Creelman, Chair of Argyll & Bute CHP Committee. The Steering Group will be supported by two sub groups, focused on workforce development and public involvement. The health sub group of the community council have been asked to nominate a representative for the Steering Group and each of the sub groups. They will also work with the CHP public involvement manager to identify groups who will be invited to participate in the public involvement group.

·         The GPs have formally notified the CHP that they are now in a position to be able to continue providing GP out of hours services for Port Ellen and Port Charlotte areas. Letters have been sent to all patients registered with the two practices notifying them of this change.

·         The building works to create two new out patient consulting rooms are now complete.   Work is underway in the physiotherapy department and will be completed before the end of March.

·         The dental van will be returning to the Islay Community Hospital site by the end of April. Building work is due to commence this week in the car park on a pedestrian crossing in preparation for the return of the van. The connection to  services will be undertaken by contractors in 6-8 weeks time.

 

Mid Argyll

 

·         Further locum GPs will be secured for Inveraray/Furnace GP practice for the period March-June 2013 to ensure service continuity, while we await a decision by Scottish Government regarding a proposal submitted by NHS Highland to test models for sustainable remote and rural health care services. One of the community council representatives has been updated so that the community are kept informed of progress with the GP vacancy.
