Joint Community Care Plan (2002 - 2005)

This plan covers the three year period, 2002 - 2005 and aims to improve the delivery of Community Care Services. It gives a brief explanation of the planning framework for Community Care at both national and local policy level and examines the Community Care financial provision and future joint planning initiatives for individual care groups.
